Schools nomination forms are to be filled out by a school member nominating one of their students for a scholarship.

STUDENT FORM

Student forms are to be filled out by the student nominated by their school.

Students must be attending a postsecondary institution that is a member of the California Association of Private Postsecondary Schools (CAPPS). Any entry without a corresponding CAPPS member school nomination form will not be considered for the scholarship.
